Annette,
Thanks for your answer. I know that the instructions for the index does not
mention putting page numbers, but that's the purpose of an index, to find
the page on which the name is discussed. That's why I asked the question.
So to be clear, you are saying that we prepare an index of names
(alphabetized, of course) but we DON'T put a page number beside each name.
Right? The publishing company will put the page number.
