Hi Fellow D-OGS & TMG members!

I'd like your help with CensusMate. [I know you all do a good job of recording your sources when you find data ...]

I want to put a form on the back of the CensusMate worksheet for users to record the source, registry, line numbers, etc. where they found the data for each census year.

Do you know of an available form to record all this data that I can "research" to extract a worksheet for CensusMate users?
