Would all that form(ality) apply?

Most genealogists would be wanting the (slightly) cheaper
micrographic 'photo' copy rather than official BDM *certificates*

Re ordering NZ BDM certificates, you certainly do not need the folio
number as they now order by entering the details on computer (the data
was entered by EDS Upper Hutt from the original bound volumes; ie
the RG Head Office volumes, not the District volumes).

The form BDM 93 "Request for Birth, Death or Marriage Certificate"
asks for:

Birth Certificate:
Certificate Details (surname, given names)
Date of Birth
Place of Birth
Mother (surname, given names)
Mother's Birth Details (surname, given names)
Father (surname, given names)

Death Certificate:
Certificate Details (surname, given names)
Date of Death
Place of Death
Name of Spouse if Known (surname, given names)

Marriage Certificate:
Bride (surname, given names)
Groom (surname, given names)
Date of Marriage
Place of Marriage

They ask you to "show exact date if possible" but will order with a year
only (and in practice will search a year either side if necessary I
think),
but for an extended search of a range of years (eg 1925-35) you pay
extra search fees (ring the freephone number for the search fees)!

If your cousin has difficulties ordering in the Christchurch office, s/he
could order by the freephone number (NZ only) 0800 22 52 52;
or phone ++64 4 382-3400 (from outside NZ).
