Due to a great deal of popular request, here's the editorial information
I have for the booklet:

I believe you can write them directly and order from them. I don't have
their exact address, but it shouldn't be hard to find. I can't remember
how much the booklet cost, but it was very little indeed. The booklet
is in paperback form and contains some 100 pages. (The quoted page
numbers are those of the original records, not the paperback facsimile).
