Hi Michael. I don't have personal knowledge of PA like Rich Nichols, but you
can download the form for a death cert. at the PA DOH website. Just follow
the links for 'request by mail'. In part 2, there's a box to check for
'genealogy'. There's also a drop-down menu for Gen. Requests under Birth &
Death Certs. The cost for a cert. is extremely reasonable...$9.00...as long
as you have the name and year of death. Do you have this info? If not,
you'll pay a lot more for a multi-year search. The multi-year search is a
separate form and you would need the County of death. I know these State
websites can sometimes be quite overwhelming, but you might want to go back
to the site and re-read everything on it. It's really quite informative and
should help you decide what to do next. Good luck.
