NARA has their own site with information about what forms you need to order what kind of records and how much the various records will cost to have copied.
It is at http://www.archives.gov/research_room/obtain_copies/military_and_genealogy_order_forms.html
If the url splits over two lines you will have to cut and paste it into the address line of your browser. Most of the forms can be requested via email. They will send them to you but the forms come by snail mail and I guess take about a week to arrive.

Hi Mitch and Ruth,
If you have a Regional Archives nearby, you can get pick up the form(s) there.
Fill out the same form twice. One for Military records and one for Pension
records, if you want copies of both.
Otherwise the address I have from an old brochure, to request the form is:
National Archives and Records Administration
Washington, DC 20408
If there is another lister who has more updated info, would you please post it
on the list, as a help.
Or, you can call the Regional Archives and they will tell you exactly what to
do. I have called my local Regional Archives asking for times they are open and
they do answer the phone.
Good Luck,
Diana
