Frank,
I know this can be very frustrating. It took me a long time to find the death of ELIZA LEIPPERT. She was on the film, but misindexed as "LEIFFERT" because of the handwriting of the clerk back in 1890.
Two Suggestions:
Make a list of all the surnames that could fall under Soundex Code D265, and check those. That may not yield much as you've already checked all the "D"s, but it can't hurt to try.
More tedious, but may work:
Think out of the box -- an old faded "D" can look like P? O? B? S? Check them all.
You may have to check the other boroughs -- All Faiths Cem. may not be correct on the location of his death.

I just looked over FHL 1324914, Death Index for all boroughs 1900-1904, for a John Deckerman (Dackerman) who according to All Faiths Cemetery died at the Kings County Hospital and was interred March 18, 1902. However, he was no where to be found in the tape, frustrating. I looked through all of the D and B's for March.

Where to next?
