All records of deaths >1948 in Greater New York are maintained at
the NYC Dept of Health in Manhattan. Where did you come across this
Queens office? It is probably the City Clerk's Office where you would
go to get >1937 marriage records, but not death records.

You can get death records for relatives from the NYC Dept of Health
for $15 each + SASE. They can be picky so I asked them to send me a
copy of their form for requesting a death record. I photocopied it
and submitted several requests for cousins, g-uncle, etc, with no problem.
They want a lot of info to positively ID the deceased (age, parents'
names, etc.), your relationship to the deceased, and a reason for the
request (genealogy is OK but I never use that word; I just say "family
records".)

Although they say they will search up to 2 years for the basic $15 fee,
if I don't have a pretty good idea of the exact DOD I usually try to
check the indexes myself. They are available through 1965 at FHCs.
