The Cemetery of the Evergreens, the one west of Cypress
Hills Cemetery is the only one there is. The address may
have changed slightly due to local road construction but
this is the place. I would suggest that you resubmit your
inquiry to Evergreens with all the new information that was
provided by the death certificate.
