The Fredonia Town Clerk would have the birth certificate. Sorry, I don't have the address. Since the birth was less than 75 years ago, you will need proof that the person requesting the certificate is next-of-kin. You may have to submit some sort of proof or an affidavit that the person is deceased. You can also order the certificate from the NY State Dept. of Health:
