Forgive me if someone already replied to your query.

Go to the "Upstate New York Genealogy Blog" (in NY-ese, "upstate" = anywhere north or east of NYC) and look at the information in the right-hand column. Follow and read all links and comments. As you'll see, paying $22 to a city/town clerk will net you a redacted, type-written document while paying $22 to the NYS Department of Health will net you a photocopy of the original document. (It will also depend on the years you are interested in.)
