Is there any way that somebody might communicate to me (by attachment, by
fax (301-864-5749), or by mail (5301 Baltimore Avenue, Hyattsville,
Maryland 20781-1911)) a copy of a form from the LDS entitled, I believe,
"Request for Photocopies"? I'm assuming that the form is free, but if it
isn't--and if you're willing to mail me a copy--let me know and I'll send
you the money plus a SASE.

I've got the names, and I've got the reel numbers, but not the form.

Failing that, can somebody point me to it on the web? I've looked for it on
familysearch.org, and on cyndi's list, and in a couple of other places, but
I can't find it--not even an address at which to request it.

Thanks a lot for any help you can offer. I am, for the first time, hot on
the trails of TWO great-grandfathers and I don't want to let them get away
from me for want of the proper form. These guys are my Manhattan
great-grandfathers, not my Staten Island great-grandfathers, but I've been
cluttering up the NYC list lately with a lot of requests for help, and I'm
embarrassed.
