I believe the cemetery you are referring to is the Smithtown Cemetery - just north of the intersection of Rte 111 & Rte 25. I am not sure whether is is affliated with the church at that intersection. You might try the Smithtown Historical Society at www.smithtownhistorical.org - they are located a few doors down from the cemetery - maybe they can give you some guidance.
