If the death occurred within the borders of the Village of Patchogue
(incorporated in 1893) in the Town of Brookhaven, then you would want to
contact the Village Clerk for the Village of Patchogue.

If the death occurred outside the Village of Patchogue but within the
Patchogue, NY postal zone, then you would want to contact the Town Clerk for
the Town of Brookhaven.
