Annie,

Since your mom has Alzheimer's (mine had it too) I assume you or some member of your family has Power of Attorney. When I first started working on our family mom was still alive and when I wrote for her birth certificate I was told that I needed a death certificate unless I was requesting my own birth certificate. Since I had Power of Attorney I signed my mothers name followed by mine with POA after my name and sent a copy of the POA papers and was able to get my mothers birth certificate this way.
