Below are links to the different health departments I found. I think you
will have to go to the County Health Dept if they died outside Hamilton or
Middletown. If they died in either of those cities, you need to go to the
local health departments.

You may purchase a certificate in person or via mail. There is a statutory
fee of $15 per certified copy of a birth or death certificate. No personal
checks will be accepted. To download the birth and death certificate
applications please visit our Forms page on this website.

I can't find a website for the Hamilton Health Dept. I believe they are
separate from the Butler Co. Health Dept. If anyone know what it is,
