Hello,

I am needing some information on where to go or to call for a certified
birth certificate for my mom, she was born December 13, 1941 in Coshocton county,
Ohio.

Need this information immediately, we need it for filling out paperwork for
a nursing home.....she has the disease ALS, otherwise known as Lou Garigh's
disease........not to much more time for her.

Any help is appreciated.
