I am not trying to be sarcastic - but you must not have been dealing with
the Diocese for the last ten years. If they want a form filled out, am more
than happy to do that. Didn't even know there was a website and an actual
request form! This must not be widely known information, or I would have
done this a long time ago. Thank you for the information.
