Hi All!
This is the info I received from the Ohio Health Department in response to my query regarding 1900 and on birth and death records.

You may use the standard Certified Copy application (found on their web site)

to make uncertified requests.

Write "UNCERTIFIED COPY" in large letters

someplace at the top of the form.

The cost of uncertified copies is three cents

per copy.

In addition to the copy cost,

you must either send money for the first-class postage

($0.34 for 1-5 copies) or a self-addressed, stamped envelope

plus three cents for each copy up to five.

(I recommend sending the extra money, as there is

the remote possibility that the SASE could become

separated from the request somewhere along the way.)

For more than five copies, the postage is as follows:

6-13 copies, $.55

14-20 copies, $.76

more than 20, contact ODH at (614) 466-2531

Make your check or money order payable to "Treasurer, State of Ohio."

Mail your request to:

This address is not quite the same as what appears on the application

but my understanding is that this is the current preferred address, and it

DOES appear elsewhere on our web site.

PLEASE NOTE!! As of October 1, 2001, the fee for a certified/legal copy of a birth or death certificate from the Ohio Department of Health/Vital Statistics will increase from $9.00 to $10 per copy. This will not affect requests that have already been accepted before that date.
