The format is the same as used by OGS and is as follows (and this may well be the way you are doing it):

Surname, First Name
DOBirth: 1-Jan-1904 Place: Norwalk, OH
Marriage: 1-Jan-1924 Place: Norwalk, OH
DODeath: 1-Jan-1983 Place: Detroit, MI

Spouse - Maiden Name, First Name
DOBirth: 1-Jan-1912 Place: Detroit, MI
DODeath: 1-Jan-1976 Place: Detroit, MI
You can list the first names of the children here if you really want to.
Your contact informantion here-
______________________________________________________________________

Spouse - Maiden Name, First Name
See: (give husband's last then given name)
(This card is used as a cross reference)
Your contact information here -
______________________________________________________________________

If this does not give you enough room to put your contact information in the bottom right corner of the card, put "contact info -arrow-" in the lower right corner and put your information on the back of the card.

Do not try to put all children's information on the parent's card. Make a separate card for each child. Making cross reference cards for each female child that has married.

Also send in a five generation chart along with your cards.
