Death Records
The Ohio Department of Health houses only the death records of people who
died in Ohio from January 1, 1945 to the present. If the death did not occur
in Ohio, contact the vital statistics office in the state where it did occur.
Certified copies of vital records may be ordered by mail or in person and
cannot be ordered by E-mail or telephone. There is up to a six month delay
before records of new deaths become available to order.
To order a death certificate from the Ohio Department of Health's Vital
Statistics Unit, click here to download an application. (The free Adobe
Acrobat Viewer is required to print this application. Click here if you do
not have Acrobat installed on your computer.) Otherwise, mail a signed letter
that includes this information:
Step 1. Information about the person whose record is being requested:
full name of decedent
date of death
city and county of death
Step 2. Mail a check or money order drawn on a United States bank and made
payable to the "Treasury, State of Ohio." The fee for a certified copy of a
vital record obtained from the Ohio Vital Statistics Unit is $7 per copy, per
record. Do not mail cash. Mail the information described above along with the
fee to:

Step 3. Allow 4 to 6 weeks from date we receive your request for delivery. We
cannot provide same day service.
Special Note: If you do not know the date or place of death, you may request
a search of the state Vital Statistics Unit files and records. The fee for a
search is $3 for each ten years searched, for each name searched. Payment
must be made in advance. After searching is completed, you will be informed
if the record was located.
You may telephone the Ohio Department of Health's Vital Statistics Unit by
calling (614)466-2531. This line is answered by a recorded menu-driven
message system that offers general information on office hours and specific
information on how to order documents. A staff person will speak with you for
further information if you stay on the line after selecting an option and
listening to the recording
