I believe I found your Uncle Frank on 2 websites on Ancestry.com. They are as follows:

>From the Ohio Deaths 1958-2000 site:
Frank Costanzo, born May 14, 1918 in Cleveland, Ohio
died Feb. 13, 2000 in Willoughby, Ohio, Lake Co. Age 81 at the time of his death.
Widowed
Mother's maiden name-Trusso
Served in the Army.
Certificate number 013370

>From the SSDI:
Frank J. Constanzo, born May 14, 1918, died Feb,15, 2000 in Willoughby, Ohio, Lake Co.

The dates of death are different by 2 days but I believe this is the same Frank. I hope this helps some.
