You need to write to the Bureau of Vital Statistics in OKCity. I have the
address, but not in front of me. Let me know if someone else doesn't show
up on here with it. I'll start digging. :)

Also, there are numerous Vital Statistics websites that offer forms to fill
out, and you can order them there, too.

OK is funny about giving our birth and death certificates...and will many
times ask you for identifying information. But, with it being 1906, I doubt
there'll be a problem.
