They didn't use a specific one. Here's what his membership certificate
says...

.....(Chelsea)....., Oklahoma and the dependent members of his family
consisting of the following persons:
Name......(Martha Jane Johnson)...Age:..(61)
Name......(Bertha Viola Johnson)...Age..(20)
Name......(Helen P. Johnson)......Age...(17)
are members of the Community Burial Association and as such are entitled to
benefits provided and set out in the Articles of Agreement printed below:

There are 5 articles listed with this certificate, if interested I can post
it.
For my great Grandpa he received benefit's of $50 at the time of his death,
which was 1943 & the certificate was applied for in '42.
His funeral cost $131.50 part of it was paid from the Burial Association &
the Military & one of his kids.
The funeral home he apparently used was the R.W. Benjamin which I believe
has changed names, I'd just contact one of the local funeral homes to see
what they tell ya about it.
