I've had the same problem! I sent off a request in November 1999. The check
cleared January 2000. Nothing. I wrote a letter and reprinted the letter of
the individual I wanted. I also included a copy of the check, endorsed. I
did this in March and still no answer. I would love to know where/who to
call about this. It's a little frustrating that I haven't even received a
generic letter or a phone call about this.
