I searched a wonderful book that I own for pictures of the gravemarker
described, but could not find any images. However, there is a fabulous
bibliography that might be worth searching to find what you're looking for.
Hundreds of articles about gravemarkers. I'll bet there will be something in
one of those articles that will help you figure out what you're looking for.

The book is:

Richard E. Meyer, ed., Cemeteries and Gravemarkers: Voices of American
Culture (Logan, UT: Utah State University Press, 1992).
