Go to the site <vitalrec.com>. When you get there scroll down to the state
of Pennsylvania and click on there. This will give you addresses, forms to
use, and fees.
I don't think your 1904 death would be recorded with the state, but probably
will be on a county level. You can get the address for Allegheny County
there, also.
