Hi -
I have requested over 40 applications (my family - all sides, as well as
my husband's). I use a form letter approach sort of like this:

Freedom of Information Officer
4-H-8 Annex Bldg.
6401 Security Blvd.
Baltimore, MD 21235
Dear Freedom of Information Officer,
Please send me a copy of the application for a social security card
(Form SS5) by the following person. I found the information on the
Social Security Death Master index.
My understanding is that the fee is $7.00 if I supply the social
security number, or $16.50 if the social security number is unknown or
incorrect. Enclosed is a check or money order for $___________, made
payable to the Social Security Administration. Thank you for your
assistance.

Sincerely,
Address:

If you don't know the birth and death dates, but just know the SS#, it
is ok but you might get someone else by mistake. They will re-research
for free if it is a mistake.
I have been going to the FamilyTreeMaker website or the Kindred
Konnections website for the Social Security Death Index, searching by
names then filling out this form letter and sending it with my check.
Shortly thereafter I get the copies from their microfilm.
