Maggie,
I located a few death certificates from Pennsylvania,last year.
They were from1909,1912 and 1920 and space for both parents is provided on
the form.
Also country of birth and occupations for parents and the deceased.
These are state issued death certificates that I ordered from Vital Records
at New Castle,Pa.
