Could I have the refresher course in Pennsylvania death certificates?

I am primarily interested in the pre-1900 era and would like to know what
questions were asked about parents, spouses, and maiden name for married
women. Were those questions always asked or added to the certificates at
some time?

And what is the best (quickest, least expensive) source for certificates?
Can they be obtained from different levels such as state, county, or town?
