Subject: [PBS] two birthdates

Once again I'm asking for your expertise. I'm going to write a letter to
the parish (Catholic) where I think my ancestor's family were parishioners.
I'm using Rosemary Chorzempa's book POLISH ROOTS, taking paragraphs from her
chapter, "Writing Letters To Poland" and adding the diacritical markings by
hand.
Problem: There are two separate birth dates from the records I have to
date, one from the marriage certificate and one from the Declaration of
Intention - kind of puzzling since you'd think he would have given that
information directly in both instances.
When I fill in the line, "data urodzenia," shall I put both dates with
question marks after each; or, is there a Polish phrase that means
"uncertain which is correct" that I can add so as not to confuse the
reader. Any other suggestions? All help appreciated.
