My mom is a special friend to my uncle who just died. They were originally related by marriage, but that hasn't been true since 1968. Special friend sounds better than "ex-sister-in-law who took care of him in his dying days." They had no other relationship, on paper or otherwise except lifelong friends. My uncles brothers wanted to acknowledge her care for him by using some term.

It's a good question though.
