Inside SA, you apply to the Dept. Internal Affairs (if it still has that
name).

Outside SA, contact your nearest local SA Embassy or Consulate by phone. They
will send you forms, extract money, and then you wait. You may wait for up to
a year, but my local consulate (Chicago) has eventually come through on all my
requests.
