I'll put my two pence worth in!

Both myself and my Grandmother have/were married three times. (I blame the
genes and bad men!) My Grandmothers third marriage certificate was Lily Name
formerly Name Nee Name. Mine just gives the one name, my maiden name because
that is the name I have always used. My children's birth certificates
(different fathers) show the one name I have always been known as. My
Grandmothers children's certificates (again different fathers) give the
married name at the time of the child's birth and no others.

Registrars, although given the format as to what they are supposed to enter
on certificates all had their own little foibles and so it is best to keep
an open mind, assume nothing and investigate all possibilities.
