Check to make sure you attached the repository to the
source. If this isn't it, then look at the repository entry
and make sure there is text in the Other field (this is what
will print for [REPOSITORY]) and the address fields (which
is what will print for [REPOSITORY ADDRESS]). If nothing
else, edit your source definition through tools->options and
make the repository and repository address optional by
enclosing each of them in < > (although you should have the
repository for marriage records).

Here is how I do a civil marriage record in going along with
Mills formatting:

Full Footnote: [TITLE]< [VOLUME]><: [PAGE]>, [REPOSITORY
ADDRESS]<. [CD]>
Short Footnote: [TITLE]< [VOLUME]><: [PAGE]><. [CD]>
Bibliography: [REPOSITORY]. [RECORD TYPE].

Where in my Source entry screen I have something like the
following:

TITLE: Hancock County Marriage Record Book
RECORD TYPE: Marriages
VOLUME: 4
PAGE: 42

And for the Repository, the following:

OTHER: Indiana. Hancock County. Greenfield. Hancock County
Building.
Clerk
ADDRESSEE: Hancock County Clerk
CITY: Greenfield
STATE: Indiana

Which produces the following output:

FULL FOOTNOTE: Hancock County Marriage Record Book 4: 42,
Hancock County Clerk, Greenfield, Indiana<. [CD]>.

SHORT FOOTNOTE: Hancock County Marriage Record Book 4: 42<.
[CD]>.

BIBLIOGRAPHY: Indiana. Hancock County. Greenfield. Hancock
County Building. Clerk. Marriages.

Keeping in mind that the "<. [CD]>" is for the citation
detail which may or may not exist depending on whether I
have entered it.
