Have also had the same problem.
Some 28 files are involved, most of which are in the Apps sub-directory.
Having applied the 4 to 4a patch can I now go back and install version
4.0 & upgrade direct to version 4.0c.
Does this not have an implication as far as my data files are concerned
as they are of course now associated with version 4.0a.
