I always include the cause of death somewhere if I have the information.
If I want to call attention to the cause of death, I include it in the
memo field of the Death tag. I always include it in the Citation Detail
so that it is available to those who may have a personal interest in it,
say for family medical history purposes. As always, you need to
anticipate the needs and desires of your intended audience, many of whom
you have never met. I have a granduncle who was murdered on Boston
Common, immediate cause of death: fractured skull. I don't think it
would be appropriate to omit those facts, nor the many newspaper articles
ensuing.
