Did you try the following template?
"[Title][GivenName][Surname][Suffix]"
Note: I left the spaces out between the names.

I first created a Name Tag called "Name-MR" to record the actual name
shown
on a marriage record. I next created a name style called "Name-MR" to
output the name in quotation marks. The Output Template reads:
"[Title] [GivenName] [Surname] [Suffix]"
The addition of the quotation marks seems to interfere with the
conditional
quality of the fields: a space appears before the GivenName (if no title
is
entered) and after the Surname (if no Suffix is entered).
Using "-" or "--" in the unused fields has no effect; the unwanted
spaces
remain.
I couldn't find any guidance in Help on this, but I'm hoping there's an
easy
way to get the quotation marks without having to create a Name Style for
every possible combination of fields.
