To document (or cite as a source) a passport application, you would create
a Source record just like any other source document.

The appropriate one. If the data being referenced is birth data, then the
Birth Tag. If the data is the present residence, then the Residence
Tag. The Name Tag, marriage information and other Tags may apply as
appropriate.

Well, since Ms. Mills does not include a specific example of a model for a
passport application you will need to select a Source Type that most
closely (in your opinion) matches the source document in question. I would
suggest the following:
-- Government Document (Published)
-- Manuscript (Unfilmed)
-- Naturalization Record

There are many others that might also fit your needs (including even
Lineage Application). You may want to modify the default Source Templates
on the Output Forms tab of the Source Definition Screen in some
cases. Alternatively, you could select one of the standard Source Types
and use it to create a Custom Source Type.
