Alice,
I immediately see a problem with your sentence, though not one relating to
your question. When you say "BIRTHMOTHER put her child up for adoption and
she was adopted by ..." the "she" grammatically refers to BIRTHMOTHER, not
to her child (who for that matter could be male!). How about "ADOPTEE was
adopted by ..." After all, people can figure out that the birth mother put
the child up for adoption, assuming that it isn't confused with a case in
which the birth mother died.
Could you make the mother a witness to the adoptee's name tag? (I've never
tried making a witness to a name tag.)
