To sum this up...

There is not a custom.tbr file to start with.

If you create custom toolbar(s), the code expects to find at least one
custom toolbar named custom.tbr.

If you create your custom toolbars and never save one as custom.tbr, the
problem of custom toolbars not working correctly as has been described in
this thread happens.

If you have created custom toolbars and have saved them but have never saved
one as custom.tbr, the solution is simple. Copy any custom toolbar file to
custom.tbr. You never need to use the one named custom.tbr. It just needs to
exist.

We have asked that this issue be fixed.
