Darrell,
I mostly avoid that problem (and it's definitely a problem) by including
only the county and state place levels in the index. The only drawback I
can see is that preposition is part of the underlined links on my person
pages, which looks a little odd, but isn't a major issue. Most of my places
are county,state anyway, in which case I always use "in" for the
preposition.
