The problem is the Agency Source Element is in the Author Group. That group, like all "people" groups, expects commas to separate surnames and given names, and rearranges the name part appropriately for the various notes.

The solution is either to agency name without commas, or change that Source Element to a different one, one not expecting people's names. Or, you can type the name of the agency directly in the templates in place of the [AGENCY] source element.
