The primary name does not have to be her name at birth. That's the usual
case, but there are good reasons to use another name, and this is one. My
suggestion is to change her primary name to Dorothy Albert Duncan. For any
events where you want to refer to her as Mary Louise Hewins, use the feature
in TMG where you can choose which name record to assign to an event. So, for
example, you might assign Mary Louise Hewins as the name record for her
birth and baptism events, for the adoption event, etc., but leave all events
that occurred after her name was changed using the new primary name.
