Mary,
In fact I obtained a certified/true copy of a death certificate from
the county health department about two weeks ago. When I got home and started
going through the information I got from them I found that my Great
Grandmothers death certificate 'date of death' was recorded as being 100 years
after she had actually died. I am glad it was on someone that I knew the
correct date for, otherwise it could really lead you astray. I went back and
they gave me a new copy that was correct.

A photocopy or a microfilmed copy would be an image copy of the original
record.
I would consider it an original source. However, cite where it came from
[photocopied by whom, what microfilm # and repository] for you may find
out
later portions of it are missing or were not microfilmed. This can
influence
further evaluation of the evidence.
A copy created by hand or typewriter is derivative.
