I don't know what other's do, but here is what I do.
I've photograph headstones in small cemeteries. Then use NOTEPAD and I
transcribe the inscriptions, indicating which photo-file goes with
which inscription. I make sure the name and location or address of the
cemetry is at the top of the page. I, then, zip the photo files and the
text file together. I usually do them in groups of ten. I submit them to
Teri Brown at the Historic Texas Cemeteries website at
http://freepages.genealogy.rootsweb.com/~txcemeteries/. Teri does a very
good job of setting them up and it's quick and easy.
I also upload them to http://www.findagrave.com, but that is a much
slower process and must be done one at a photo at a time.

In addition to the headstones, I try to get a photo of the entrance to
the cemetery, any Texas Historic Markers, and then a scenic/landscape
shot of several of the graves.
In addition to still photos, I now set a video camera on a tripod and
video each headstone as I read instcriptions out loud. Playing the
video tape back really helps when I am trying to transcribe the
inscriptions.
I sometimes give a copy of the video to what ever little society or
museum is associated with the upkeep and preservation of the cemetery.
On the project I am currently working on, I may also put the
information and photos into a doc file or pdf file, store them on a CD,
and give that to them also. It's not that much more work.
