You need to obtain the form, "Ship Passenger Arrival Records (NATF 81)," listed on the webpage and then mail it in with the appropriate fee. You might want to consider doing the search yourself on microfilm at a Family History Center. It will save you a lot of money and you can view the entire manifest rather than just the page your ancestors are on. See here for microfilm roll numbers...
