Unfortunately many mining companies don't obey those modern health and
safety regs.
The majority of "accidental" disasters are actually due to the deliberate
neglect of those regulations.

The one you may be thinking of in the US occurred last January in Sago,
Upshur Co. WV near my husband's hometown.
There was a severe thunderstorm that went through that morning at exactly
the same time the explosion occurred. There were lightening strikes
verified all around that mine and in the sealed off area where the
explosion happened... there was no source of ignition.
It is believed that an unusually strong lightening strike fueled by methane
gas that naturally accumulated in an abandoned area of the mine caused the
explosion. This particular mine was operating under a number of safety
violations at the time. There's no telling if this disaster could have been
prevented had they kept abandoned areas monitored and properly vented.
